I've known a few people with Type 1 Diabetes, and low blood sugar can lead to a change in personality fairly quick.
Yupp. My little sister has it, and she can sometimes have some pretty intense mood swings. Sometimes anger. Sometimes weakness. Sometimes irritability or feeling uncomfortable.

Of course he's nervous, 95% of the world would be nervous if they were dropped halfway around the world onto a major city that speaks a different language, at the age of 18 with cameras/microphones aimed straight up their butts.

Moving to New York City at 18 years old is crazy. The frame of reference for your entire reality goes right out the f***ing window. He's not gonna just hop on a subway and leave the station catcalling and bitching about the Mets.

You say this like either of these two traits is characteristic of being a real New Yorker...

Is it just me or does he seem REALLY shy? Maybe nervous?

That's most probably just a cultural thing - what seems pretty normal for a Finn can easily feel like super introverted and shy to an American. We don't tend to thrive when it comes to small talk and smiling like crazy in random social situations with strangers. I have never thought that he would be especially shy for a young Finnish player, he's like totally unremarkable in that regard. Shows his emotions in public only when unguarded, like that "vitun vittu" to himself when he missed an open goal in the WHC (something like "f***ing f***" - ok, well effing f then).

His most sincere smile since he was drafted, that's for sure. lol

Yup, that's actually a genuine smile

I do think that Kakko seems a bit reserved even for a Finn. Of course he's not comfortable with the language yet but he's not the most outgoing personality type even in Finnish.

But I don't think that's an issue at all. He is super competitive on the ice and that's all that matters.

He might actually be a good fit for the Rangers. He's not the type of guy to get his head swollen by the bright lights or get distratcted by all the temptations that the city that never sleeps can offer to a kid who has some money in his pockets.

From everything I've heard Kakko is not shy at all. He's propably just guarded when put in this new situation and doesn't want to do anything "stupid". Reminds me of when he used the translator for interviews and people deducted his english must be horrible when in reality it's him just wanting to give a better interview.

He's gonna be a Yankees fan.

Yeah, I wouldn't describe him as shy either. I think he just likes giving different situations a little bit of thought before rushing into them. A lot of analytical perfectionist types are like that, especially in Finland where extrovertism is not that much of a cultural norm.

Turku is not a small town, man

Turku is pretty small. Maybe in Finland it's considered on the larger scale but a population of 180k doesn't even crack top400 in Europe. It'd a small town. NYC has a population of 8.6M, the whole of Finland has a population of 5.5M. NYC has a population density of 10,200/km2, Finland has a population density of 17/km2.
