Late specialist appointments usually means they were exhausting all options to recover with more conventional recovery techniques and they're now at the point where they're consulting a surgeon because the conventional methods weren't resolving the issue.

The injury news is never good for the kid. No doubt we all hope he finds his way back to good health, but the odds seem to just get longer and longer.
