I think it has to do with whether the ref believes they were “coincidental.” So, sometimes a guy might slash a guy and then a different teammate might come over to help out and get a penalty. These are two separate events thus 4 on 4.

When Tkachuk seriously injures somebody it will be on the refs. They consistently allow him to throw cheap shots and then hide behind them.
