While that is a pretty big **** up, people need to remember when you **** up in the media, everyone sees it. If you attach the wrong file in an e-mail to a co-worker, only you and the co-work know about the **** up, or maybe a few other he/she might have told. You attach the wrong file to a clip in broadcasting, potentially millions of people see it.

The media in general doesn't make to many mistakes like this, but when they happen they are glamourized and shared everywhere, which gives the impression they happen much more frequently than they do.
