The rule is that no penalties can be reviewed. Exceptions were made for major penalties and double minors with a couple high profile blown calls since they can impact the game so much more. If you don't have it set up like that then the expectation is that every call winds up reviewed or it's unfair and blah blah blah.

Reviews slow the game down too much already. The better solution is Rod's eye in the sky ref idea who can watch in real time, potentially including replays, and call down missed calls or corrections to the on ice refs.
