Manoah hasn't even thrown a pitch in full season ball in the minors. I think him being a pen option is realistic because he can just use his best 2 pitches which might have been MLB level as soon as he was drafted. Despite him dominating in the spring him being an MLB starter already feels way too optimistic to me.
