Do you have an example or some sort of clarification? The official rules state it's an automatic suspension and I've never seen that not be the case.

While I am certain there are 5-6, probably more slew foot penalties over the past two seasons for which extra games were not served, the only name and time period from recall is Dvorsky right at the Christmas break. I think it was the first of a home and home with Barrie.
