It does change their 'worst case' scenario, but you are largely correct. Given that the first four picks are in the NBA lottery, finishing worst gives you the 5th pick at worst. If they finish second worst, then they could pick as low as 6th, and so on.
I still hate the way the NBA draft is done. Top pick or two, fine, but the worst team in the NBA should be guaranteed higher than the 5th overall pick. Glad the NHL doesn't do it that way.
