Pretty sure OTT tried to challenge and it took the Caps pointing out that they already challenged and failed earlier in the game so they can't again.

I thought it was only one per game.

I think the rule is if they're successful they retain the ability to

I think you can challenge as many times as you want. It's a minor penalty if you're wrong so no need to limit challenges.
