he is responsible for managing the bench. if too many players went out that means there was a mis communication or he did not make something clear enough to the team. if he says it was his fault, i'm inclined to believe him.

Managing the players on the ice is a pretty big responsibility for a head coach.
And he did say something to the ref that caused the abuse of officials penalty.
