If they need him in there for toughness (that is another debate) please play him at forward.

He should not see the ice again but tough to play 5 d-men.

Not on this team. That's a move you do if you expect, honestly expect, something more of the group. On this team, you throw the guy that messes up out there again. See what happens. If he messes up again, give him another chance.

Same reason, even if the team has a chance to win a game, Kopitar and Doughty shouldn't constantly be out there. I get why TM does it, he's a professional coach, and his job is to win the game, but these are the years where you just see what happens.

Problem is, those are plays you expect every player to make! Stopping plays by intercepting pucks, and making breakout passes should be the bare minimum for a defenseman.
